'The Patient' Finale Prompted My Loudest Screams of the Year
Alan threatens Sam’s mother, Candace, with a sharpened tube of foot cream, demanding Sam call the police. Sam refuses, believing Alan is bluffing. When Alan cuts Candace's neck, Sam snaps and strangles Alan to death.
"I—" (Ezra’s first word in his own therapy session).
Before he dies, Alan experiences a "mental escape"—a dream sequence where he is safely home with his family. The Resolution:
"Call emergency services. Call them now, or I'll kill her."
In a twisted application of Alan’s lessons, Sam realizes he cannot stop killing on his own. He chains himself to the floor in his basement and gives his mother the key, effectively imprisoning himself.
"My therapist told me not to kill you" (to his father).
Sam leaves Alan’s body to be found and sends a final letter to Alan's children, Ezra and Shoshana. The series ends with Ezra beginning his own therapy session, suggesting a path toward healing and breaking generational trauma.
