The story centers on , a "Spartan" outpost at the southern edge of the Metro that is under constant siege by mutants. When communication with the central Metro is mysteriously cut and supply caravans stop returning, a small group sets out to investigate the threat:

They soon discover that Tulskaya Station has been infected by a deadly, highly contagious plague. Hunter, traumatized and ruthless, believes the only way to save the rest of the Metro is to "sanitize" (massacre) everyone at the infected station.
